Pathways To Care: pathways2care.net
Pathways to Care Network is a not-for-profit network of health care providers working to decrease barriers, to educate the community, patients, and providers, and to increase collaboration to provide quality care. They needed a website that would act as an information repository as well as a point of contact for inquiries to joining or supporting PCN.
